Phase 2 PaTH Forward trial evaluated palopegteriparatide in adults with chronic hypoparathyroidism, including a 4-week randomized, double-blind, placebo-controlled period followed by an open-label extension up to 266 weeks.
59 participants enrolled; mean age 50 years, 81% female, 92% White, with most cases acquired from neck surgery.
Efficacy outcomes
93% of participants were independent from conventional therapy (no active vitamin D, ≤600 mg/day calcium) at week 214.
98% maintained normal serum calcium at week 214; mean 24-hour urine calcium normalized within 26 weeks.
Bone turnover markers (CTX and P1NP) and bone mineral density (BMD) scores remained stable and within normal ranges through week 214.
Renal function outcomes
Mean eGFR increased by approximately 9.6 mL/min/1.73m² from baseline and was sustained through week 214.
67.8% of participants had a clinically meaningful ≥5 mL/min/1.73m² increase in eGFR at week 214.
