BHB/HRS-1893 is a selective, reversible cardiac myosin inhibitor designed for rapid action and ease of use in symptomatic obstructive hypertrophic cardiomyopathy (oHCM).
The study aimed to evaluate efficacy and safety of multiple dosing regimens in adults with symptomatic oHCM.
Current CMIs require complex titration and monitoring, limiting broader adoption.
Study design and patient characteristics
Adults with symptomatic oHCM, resting LVOT-G ≥50 mmHg (or ≥30 mmHg with Valsalva ≥50 mmHg), LVEF ≥60%, and NYHA Class II-III were enrolled.
Three dosing regimens were tested: 20-40-60 mg BID, 40-60-80 mg BID, and 40-80-120 mg QD.
Baseline mean age ranged from 48.4 to 54.8 years; most patients were male and on beta blockers.
Efficacy outcomes
Rapid, deep, and reversible reductions in LVOT-G were observed across all dosing groups.
86% of patients in the 40-60-80 BID cohort achieved Valsalva LVOT-G <30 mmHg; 88% maintained this at week 39 in the extension period.
Functional capacity and symptoms improved, with increases in peak VO2, KCCQ-CSS, and NYHA class improvement in most patients.
NT-proBNP normalized in over 80% of patients, indicating improved heart failure status.
