21 Oct, 2026Positioning and pipeline
Ersodetug is a fully human antibody that allosterically modulates the insulin receptor, reducing excess insulin or IGF-2 signaling; administered by IV infusion.
Two late-stage programs target refractory hypoglycemia in congenital HI and tumor HI; GI-surgery-related HI is a potential pipeline expansion.
Phase 3 sunRIZE is complete, with its open-label extension ongoing; the Phase 3 upLIFT tumor-HI study is underway.
Congenital HI and sunRIZE
Congenital HI is a severe pediatric disease; ~60% of patients do not respond to diazoxide, the only approved medication for HI-related hypoglycemia.
sunRIZE’s primary SMBG endpoint was not met, with the presentation attributing this to an early placebo/study effect and functional unblinding; several CGM outcomes showed clinically meaningful reductions.
CGM reductions were nominally significant at selected timepoints; at Week 24/EOT, the reduction in time in hypoglycemia was not statistically significant in the full analysis set or per-protocol set.
All pivotal-study participants rolled over to the extension; ~48 months of EAP exposure was reported, with continued glycemic benefit, 2% systemic hypersensitivity reactions, and mild hypertrichosis.
Tumor HI and upLIFT
Tumor HI can result from insulin-secreting tumors or IGF-2-producing tumors; severe hypoglycemia is associated with limited treatment options and may delay tumor-directed therapy.
In EAP cases, ersodetug use was associated with hypoglycemia improvement, discontinuation of IV dextrose/TPN in 75% of patients, hospital discharge, and resumption of tumor-directed therapy.
upLIFT is a global, single-arm, open-label Phase 3 study in ~16 adults; the primary endpoint is ≥50% reduction in baseline IV glucose requirements after an 8-week treatment period.
Topline upLIFT results are expected in 2H 2026; interim open-label outcomes were described as favorable.
